填写说明 NOTE（必读）：
1. 请认真核对本人信息，确保信息准确、无误；
2. 护照姓名必须与护照上本人的英文姓名一致；
3. 出生地点须填写出生城市；
4. 通讯地址是指接收录取通知书和其他相关材料的地址，如为中国地址需使用中文，非中国地址需使用英文；
5. 家庭永久地址如为中国地址请写中文，非中国地址请写英文；
6. 学历请从“高中应届”或“高中毕业”中选择；
7. 如你的护照姓名、护照号码、护照有效期有变化或与在申请系统中填写的不一致，请务必同时上传最新的电子版护照首页（彩色扫描件）；
8. 填写完成后请再次核对填写内容是否正确，提交后将无法进行再次更改。
1. Please confirm your personal information, make sure that all information is correct.
2. Please make sure that your passport name is identical to the English name stated on your passport (in Family Name, Given Name order).
3. Please provide the city of birth.
4. Your mailing address is supposed to be active to receive the admission letter and other related documents. Please provide the Chinese characters if it is a Chinese address; non-Chinese addresses must be provided in English.
5. Please provide the Chinese characters if your permanent address is a Chinese address; non-Chinese addresses must be provided in English.
6. Please indicate your level of education from the following options: high-school graduates (高中毕业) or anticipated high-school graduates this year (高中应届).
7. A scanned copy of the front page (photo page) of your passport must be uploaded in case you make any changes to your passport name/passport number/date of expiry.
8. Please make sure that your information is correct before your submission. You can not make changes after the submission.
